Registration and Equipment Forms
Westside Lacrosse accommodates a "rolling registration"
process; a completed Westside LAX registration form
and current membership with US Lacrosse is required for any player who wishes to
participate in Westside LAX program.
There are no "try-outs" for this league.
The US Lacrosse form should be completed first
and sent directly to US Lacrosse.
The sooner you submit your registration form the sooner the team
rosters can be finalized.
